3.  If it is a donation - please fill out the application and mail it with a check or money order made payable to Dean Vereen/Medical Assisted Treatment. At the bottom of the check or money order where it says For_______write in Donation. Remember we are a 501(c)(3) Non-Profit Organization and your donations are tax-deductible.
